1. A physician referral is required to be seen at our clinic, typically this will be from your family physician, pediatrician, or another doctor.

2. Please arrive 15 minutes ahead of your booked appointment time for check-in.

3. You must phone and confirm this appointment 7-10 days in advance. A voice message may be left on the answering machine. If we do not receive confirmation your appointment will be cancelled.

4. If you are not fluent in English, an interpreter must be present for the appointment.

5. You must bring your BC Services Card

6. Clinic time is very valuable to our practice. If for any reason you must cancel or change your appointment, please contact our office immediately.

7. You must give at least 24 hour notice for cancellation/changes

8. Patients that NO SHOW, CHANGE and/or CANCEL appointments without 24-hour notice will be charged a penalty fee prior to re-booking.
